It’s not really artsy, but it’s the cutest thing I’ve heard for a while.
My friend once gave his girlfriend at the time (now his wife) the best present she said she had ever received: a miniature notebook (about 2″x3″ he purchased) and on each page, he wrote one reason why he loved her. It’s a perfect last-minute gift but it means so much. Trust me, your girlfriend will be incredibly touched.
Craft her a jewelry box (and paint/decorate it if you desire)
Make her a necklace
Make a photo frame for her and stick a picture of the two of you in it.
Make her a book of poetry (if that’s your cup of tea)
etc.
It all really depends on what areas your strengths are in and what you have the patience for.

Make her a card and write her a poem or a song inside.
Buy a mirror with a wooden frame at IKEA and glue on small items that match her interests.
I’m acually carving a love heart out of wood for my Girl friend, maybe somthing like that
